    I have been using Digital Intake Forms for a month now. I customized my forms for my new patients and new injury. They can find the link for the forms at my website or I can also email the patients the link to the forms too. Patients can then fill up the information at their own pace and return back to us electronically ahead of their appointments It will save our patient at least 15 minutes waiting for our receptionist entering the information into our EHR before I get to see them. That is on top of the time they took to fill up the forms in the waiting room. New patient visit that sometimes lasts for 2 hours now can be reduced to less than an hour. I am grateful for the dependable service and convenience.

    Working with Mark is a pure joy. I always feel like a priority. He has called before to check in on me to make sure I understood something. This customer service is like a step back in time. The digital intake forms are very user friendly to set up. I actually enjoy making edits if I need to. You can design really any kind of form that you desire to make. Everyone should be using this company.

    Not only do we love Digital Intakes but so do our patients. It saves us time and the patient does not have to fill in forms when they arrive for their appointment. Mark has been amazing!! His customer service has been above and beyond. He calls to check in and see if there is anything else that would be of benefit. We have used their paper forms for years but this is so much better for all. We are very happy and highly recommend to anyone that wants their practice to run smoothly to use Digital Intakes.

    We’ve been using paper charts/forms since I started back in 1996. I saw an ad in Chiropractic Products Magazine for Digital Intakes, LLC and the first thing I thought was, I’m sure these guys want $100 dollars a month to do what we do on paper for just a few bucks. Well, I was floored when a saw they only charge $25.80 per month the first month is free, and the next 6 months are half off. These guys have created and amazing service. It’s so much more than our old paper forms. Patient fills out forms on their home computer or smart phone and we get them right when they’re done. We get patient reminds, notifications, a google map, & excellent support (ask for Marc). Awesome! It’s saves us time, and effort and the patients love it!
